Auxiliary Memorial Plaque
Many years ago the Saint Mary's Hospital Auxiliary established this Memorial Plaque to honor loved ones, friends, and colleagues who have passed away. For gifts of $100 or more you may purchase a plate to adorn this plaque. Please complete the form below and return to the Foundation.
